To be eligible for disability insurance in Ontario, you must meet the following criteria:
- You must be a resident of Ontario.
- You must be under 65 years of age.
- You must have a valid Ontario Health Insurance Plan (OHIP) card.
- You must have worked for at least 490 hours in the 26 weeks immediately preceding your disability.
- You must have lost at least 40% of your pre-disability earnings due to your disability.
Step 2: Gather the required documents
You will need to gather the following documents in order to apply for disability insurance:
- A completed Application for Ontario Disability Support Program (ODSP) Benefits form (Form 1).
- Proof of your income, such as pay stubs, tax returns, or a letter from your employer.
- Proof of your expenses, such as rent or mortgage payments, food costs, and transportation costs.
- A medical report from your doctor or other qualified health professional that describes your disability and how it affects your ability to work.
- A copy of your valid Ontario Health Insurance Plan (OHIP) card.
Step 3: Apply for disability insurance
You can apply for disability insurance in Ontario by mail, fax, or in person at your local Ontario Works office.

Step 4: Wait for a decision
Once you have submitted your application, it will be reviewed by the Ontario Disability Support Program (ODSP). You will receive a decision in the mail within 8 weeks.
If your application is approved, you will receive monthly disability benefits. The amount of your benefits will be based on your pre-disability earnings, your expenses, and the number of people in your household.
If your application is denied, you have the right to appeal the decision.
