1. Residency: At least one of the partners must be a resident of Salt Lake City.
2. Age: Both partners must be at least 18 years old.
3. Marital Status: Neither partner can be married to anyone else.
4. No Close Family Relationship: The partners cannot be related by blood or marriage in a way that would make it illegal for them to marry each other.
5. Willingness to Share Life Together: The partners must be willing to live together and share responsibility for each other's welfare.
In addition to these requirements, the partners must also file a Domestic Partnership Declaration with the Salt Lake City Recorder's Office. The declaration must include the following information:
* The names and addresses of both partners
* The date of the declaration
* A statement that both partners meet the requirements for domestic partnership
* The signatures of both partners
Once the declaration is filed, the partners will be considered domestic partners under the laws of Salt Lake City. Domestic partners have many of the same rights and responsibilities as married couples, including:

* The right to make medical decisions for each other
* The right to visit each other in the hospital
* The right to share a health insurance plan
* The right to file joint tax returns
Domestic partners also have some of the same responsibilities as married couples, including:
* The responsibility to support each other financially
* The responsibility to be faithful to each other
* The responsibility to communicate with each other openly and honestly
Domestic partnerships can provide a number of benefits to couples who want to live together and share their lives without getting married. However, it is important to understand the requirements and responsibilities of domestic partnership before entering into one.
