Visas are required of everyone except nationals of the following countries:

If you are a national of a country not exempt from Macua's visa requirements, a visa can be issued to you on arrival in Macau at Immigration Services in the airport. These visas are issued for a stay of up to 30 days. You can also apply for a visa in advance at any embassy or consulate of the People's Republic of China. If travel was arranged through a tour operator, they often also take care of visa arrangements for members of the tour.
A valid passport or other travel document such as a Hong Kong Identity Card or Diplomatic Passport issued by the United Nations is required of everyone entering Macau, whether or not they are nationals of a visa-free country such as the United States. A passport with available blank pages, valid for at least 30 days beyond the time frame of your intended stay, is a must.
According to the U.S. State Department's website, most of Macau's neighboring countries require a minimum of six months validity remaining on a passport, so if you intend to visit any other country beyond Macau, make sure your passport won't expire for at least another six months.
According to the Macau International Airport's Passenger Guide, three types of visas are available:
Individual visa: for a lone traveler for a single stay of up to 30 days in Macau. The visa fee is MOP 100, children under 12 pay MOP 50.
Family visa: for married couples or parents with children; the fee is MOP 200.
Group: groups of 10 or more will cost about MOP 50 per person.
