Minnesota fits both criteria:

Why "Old Man River"?

Nickname for the Mississippi (from the 1927 song "Ol' Man River" in Show Boat). Minnesota hosts the headwaters at Lake Itasca—where the "Old Man" begins.

Other States With Both?

Wisconsin borders Lake Superior & Lake Michigan, and the Mississippi forms its western border. Illinois borders Lake Michigan and the Mississippi. But Minnesota is the only one with the headwaters + Superior combo.
