USA Government Passport Policies

People who are applying for a passport for the first time must apply in person at a passport agency with two 2-by-2-inch passport photos, identification documents and a fee payment. The U.S. government has several other guidelines for passports.
  1. Applications

    • You must complete the DS-11 Passport Application, which asks for Social Security information. Do not sign the form until an agency representative tells you to do so.

    Proof of Citizenship

    • You must provide proof of citizenship. This can include a birth certificate, certificate of birth abroad, certificate of citizenship or a naturalization certificate.

    Identification

    • You will need to provide one form of primary identification and also a photocopy of the front and back of that identification. These forms of ID can include a valid driver's license, military ID, government ID or a naturalization certificate.

    Minors

    • Parents and guardians have to accompany children 16 and younger who are applying for passports. Along with the items mentioned in the previous sections, proof of a relationship between the adults and the children must be provided. This can include a birth certificate, adoption decree or a court order that establishes guardianship or custody. Also, parents and guardians who are accompanying minors will have to establish their own identities by presenting a primary identification document and a photocopy of that document.
