Every piece of information must be submitted under the same name. If you want to obtain your passport under your married surname, you need to submit an application completed in full; proof of Canadian citizenship (birth certificate if you were born in Canada, certificate of Canadian citizenship if you were born outside of Canada); identification supporting your married name (driver's license); two photos, one which will be signed by a guarantor; the required fees; and any travel document if necessary. If you want to have your passport in your common-law surname, you need to submit the same paperwork, but with a signed letter stating that the relationship has been going on for more than two years. If you have legally changed your name, submit the same paperwork, but with either a legal change of name document, an adoption letter, or a birth certificate or a citizenship certificate issued under the new name.
If you were born in Canada, you must provide a birth certificate or a certificate of Canadian citizenship. If you have lost your birth certificate, you can find it at the Registrar of Vital Statistics in the province or territory where you were born. If you were not born in Canada, you need to provide either a certificate of Canadian citizenship, a certificate of Naturalization in Canada, a certificate of Retention of Canadian citizenship, or a certificate of Registration of Birth Abroad. You need to include a document that supports you identity along with your application form. Examples include a driver's license, a provincial health care card, a certificate of Indian status, an Old Age Security Card, a federal, provincial, or municipal employee card, or a previous Canadian passport.
Any adult over the age of 16 who wishes to obtain a passport needs to pay C$87 for a 24-page passport and C$92 for a 48-page passport. These fees can be paid in person by credit card, debit card, a certified check or a money order made payable to the Receiver General for Canada. If paid by mail, credit cards, certified checks and money orders are acceptable.
You must bring two identical photos that fit strict requirements or the passport office will reject them. They must measure 2 by 2 3/4 inches; have a close-up of the head and shoulders; be printed on plain, high-quality photographic paper; and the length of the face from chin to crown of the head must be between 1 ¼ inch to 1 7/16 inch.
Your guarantor needs to sign the Declaration of Guarantor document. Your guarantor is someone who has known you for more than two years, is a Canadian citizen with a valid passport, and can be contacted by Passport Canada.
