You must complete Form DS-11: Application for a U.S. Passport, including your social security number, and apply in person at a passport facility or agency. Wait to sign Form DS-11 until you are at the facility or agency in front of a passport agent.
You must also provide documentation proving United States citizenship. This may be a certified birth certificate with the registrar's stamp or seal, a previously issued, legible U.S. passport (can be expired), a Consular Report of Birth Abroad or Certification of Birth, a Naturalization Certificate, or a Certificate of Citizenship. This document must be handed over to the agent to be processed with the application and will be returned to you via mail, with your completed passport, once it is ready.
A form of identification must be presented in person when obtaining a passport. This may be an expired, previously issued U.S. passport, Naturalization Certificate, a valid driver's license or government I.D. or a current military I.D. If you submit an out-of-state I.D. card you will need to bring a second form of identification. You should provide a photocopy of all identification you plan to use on white, letter-sized paper. The front and back of the identification card should be photocopied on the same side of the paper or on two separate papers.
Two identical, color, passport-sized photos (2 inches by 2 inches) must be submitted with your application. The photos should have been taken within the previous six months and reflect your current appearance.
