Form DS-11: Application for a U.S. Passport, submitted in person, at an approved Passport Acceptance Facility is required for new applicants. Renewals by mail require Form DS-82: Application for a U.S. Passport by Mail.
Documents that meet this requirement include a previously issued, undamaged passport, a certified birth certificate, a Consular Report of birth abroad, a Naturalization Certificate or a Certificate of Citizenship.
Acceptable identification includes a previously issued, undamaged passport, a Naturalization Certificate, a valid driver's license, current government I.D. or a current military I.D. You must present a photocopy of this I.D., both front and back, along with the original document.
Applicants under the age of 16 must apply in person with both parents and evidence of parental relationship. Provided one parent is unavailable, consent or the inability of one parent to consent must be presented in the form of a death certificate, notarized statement of consent or court order awarding sole custody.
In addition to the aforementioned requirements, all applicants must submit two passport size photos and the appropriate fees. If the applicant has travel plans to an international destination, he should check with that nation's embassy or consulate for entry requirements.
