What are the requirements for domestic partnership in Provincetown Massachusetts?

To establish a domestic partnership in Provincetown, Massachusetts, both partners must be at least 18 years of age, not closely related, and not already legally married to someone else. Additionally, one of the following requirements must be met:

* Both partners must have the same permanent residence address in Provincetown

* One partner must have a permanent residence in Provincetown and the other must live or work within 15 miles of the town

* One partner must be a full-time Provincetown resident for at least one year and the other must live or work within 50 miles of the town

Once these requirements are met, couples can file a Declaration of Domestic Partnership with the Provincetown Town Clerk's Office. The declaration must be signed by both partners in front of a notary public and include the following information:

* Full legal names of both partners

* Date of birth of both partners

* Social Security numbers of both partners

* Permanent residence address of both partners

* Occupation of both partners

* Length of time partners have lived together

* Statement of intent to create a domestic partnership
