Lost drivers licience in Texas who do I contact to replace?

* By mail: Complete the [Application for Duplicate License or Identification Card (Form DL-414)](https://www.txdmv.gov/sites/default/files/dl-414rev21718.pdf) and mail it to the address listed on the form.

* In person: Visit a [DPS driver license office](https://www.txdmv.gov/driver-license-office-locations).

You will need to provide the following documents:

* A completed Application for Duplicate License or Identification Card (Form DL-414)

* Proof of identity, such as a birth certificate or passport

* Proof of residency, such as a utility bill or lease agreement

* Your social security number

* A fee of $11

If you are unable to provide all of the required documents, you may be able to get a temporary driver's license or ID card.
