  1. Replacing your passport at home or abroad

      In the United States, go to your nearest passport office or acceptance facility. Report that your passport has been lost or stolen by submitting the statement of lost or stolen passport form DS-64. Request the passport application form DS-11.

      If traveling abroad, make an appointment with the nearest U.S. Embassy or Consulate. Appear for your appointment on time, with evidence of U.S. citizenship, proof of identity and passport photos. Submit the statement of lost or stolen passport form DS-64 and the passport application form DS-11.

      If your passport has been stolen while abroad, report the theft to local authorities. Contact the nearest U.S. Embassy or Consulate for an appointment. Take the police report with you to the embassy or consulate along with the documents mentioned above.

      Pay the applicable fees. Current fees for U.S. citizens under the age of 16 are $105 and for those older than 16 the fee is $135.
