  Complete form DS-5504: Application for a U.S. Passport: Name Change, Data Correction and Limited Passport Book Replacement.

  Place form DS-5504, your current, valid U.S. passport, a certified copy of your marriage license or government-issued legal document stating your name change and a recent color passport photos in an envelope.

  Mail the envelope to the address on form DS-5504.

Passport Issued More Than a Year Ago and Name Change Due to Marriage or Court Order

  Complete form DS-82: Application for a U.S. Passport: by Mail.

  Place form DS-82, your current, valid U.S. passport, a certified copy of your marriage license or government-issued legal document stating your name change, one recent color passport photo and applicable renewal fees in an envelope.

  Mail the envelope to the address listed on form DS-82.

If Unable to Document Name Change

  Complete Form DS-11, Application for a U.S. Passport.

  Place form DS-11, your current, valid U.S. passport, at least three public records indicating the place and date of your birth and showing exclusive use of your assumed name for a minimum of five years, one recent color passport photo and applicable first time fees in an envelope.
