How to Change Your Name on a Chinese Passport

A Chinese citizen's passport grants the privilege of international travel. A passport is the most important document required to enter and leave the country, so it is essential that the owner's information is always current at the time of travel. Changing your name on a Chinese passport is considered an amendment and it must be applied for in person. Make the following preparations to process your request as swiftly as possible.

      Visit the Chinese Consulate in your jurisdiction and set an appointment for a passport amendment. (See Resources for the location and contact information of the nearest Chinese Embassy.)

      Complete the Chinese Passport Amendment Application H-1 form that you receive from the consulate, using only blue or black ink. Forms completed in pencil or red ink will not be accepted. (See Resources to download the form.)

      Obtain a notarized copy of your marriage certificate or court order evidencing your legal name change. If you cannot provide these documents, you must submit at least three public records stating your date and place of birth that show the use of your assumed named. What constitutes public records varies with each state and country, but may include professional licenses, business records, or real estate documentation.

      Prepare to submit one passport-type photograph taken within the past 6 months. The color photo must be 2-by-2 inches in size taken against a light blue or white background. The offices of the Chinese Embassy also provide a photo service for applicants, payable in the currency of the country in which you are applying.

      Prepare your passport for scanning by removing any staples or protective plastic as they may damage your passport in the process. Then return to the Chinese Embassy consular to submit all of your application documents in person. The passport amendment service is free of charge unless you require expedited service.
