Gather your current U.S. passport, a recent, color photograph of yourself and a copy of a court order that provides evidence for your legal name change back to your maiden name.
Access the "Passport Forms" link on the Bureau of Consular Affairs website. If your current passport is less than one year old, click on the link to the DS-5504 forms.
Complete the DS-5504 forms on the website and print them out, or you can print out the forms and complete them by hand. Make sure to use only black ink when filling out the forms and do not forget to sign and date them. The forms should be printed on one-sided pages.
Place the completed DS-5504 forms, a recent photograph of yourself, your current U.S. passport and the court document that provides evidence for your name change in an envelope.
Mail the envelope to the National Passport Processing Center. The address can be found on the DS-5504 forms. You will have to pay an additional $60 fee if you want expedited shipping.

Complete the DS-82 forms on the website and print them out to sign and date. You can print out blank DS-82 forms and fill them out by hand if you choose. The forms should be printed or written in black ink on one-sided pages.

Staple a current photograph of yourself to your DS-82 forms.
Mail your DS-82 forms, along with your photograph, passport and court order, to the National Passport Processing Center address listed on the DS-82 forms.
