How to Change the Name on a Passport in the USA

Your passport is an important document required for international travel and is also an acceptable form of ID. If you become married or legally change your name, updating this information on your passport is necessary.

      Complete form DS-5504 (Application for Name Change) if your passport was issued less than one year ago. There is no fee for this process unless expedited service is required. If your passport was issued a year or more ago, you must complete form DS-82 and pay renewal fees.

      Furnish an original or certified copy of your government-issued name-change document or marriage license.

      If you cannot provide legal documents for your name change through marriage or court order, complete form DS-11 (Application for a U.S. Passport). Attach applicable first-time fees with your application, and submit them in person at a Passport Agency or Acceptance Facility (see Resources).

      Include two recent photos with your documents, unless your previous passport was issued within 90 days. Photos must be in color and two by two inches in size, and must have been taken within six months, full face with a white or off-white background. If you normally wear prescription glasses, they should be worn in the photo. Hats, caps and other head gear should not be worn because they obscure the hairline.

      Mail your application form, previous passport, photos and legal documents to the address listed on the application. Include renewal fees if your previous passport was issued more than a year ago.
