Download and print an application form from the British Embassy website.
Complete the application. Make sure you include your e-mail address in section 1a, and initial in the bottom right-hand corner of each page.
Have a new passport photo taken and include two copies with your application. These photos cannot be more than one month old at the time of application. The British Embassy in Washington recommends CVS and Walgreens for passport photo services. Photos must be head shots in which you are facing the camera, without anything obstructing your face. Keep your lips closed.
Ask someone to countersign one of the two photos for you. This cannot be a partner or relative. On the back of the photo, this countersignatory must write:
"I certify that this is a true likeness of [Mr, Mrs, Miss, Ms or other title followed by your full name]"
They must sign and date underneath.
Include your previous passport if you have it, along with the current application fee. The British Embassy in Washington states that you must pay in U.S. dollars. Payment must be from a Visa, Mastercard, money order or bank checks; personal checks are not accepted.
Submit your passport application, photographs, previous passport and application fee in one packet to the following address:

If approved, you should have your new passport within a month's time.
