Renew your passport by mail. You are eligible to do so if your previous passport is undamaged, was issued when you were 16 years of age and within the last 15 years. Download and fill out Form DS-82: Application for a U.S. Passport by Mail. Place this along with your previous passport, two passport-sized photos and a check or money order for $75 bearing your full name and birth date made out to the U.S. Department of State into a padded envelope. If your name has changed, include your marriage certificate or court order. Mail the envelope to the address given on the application.
Renew your passport in person at one of Minnesota's 200 passport acceptance facilities. You must do this if your previous passport is damaged, lost or stolen, if your previous passport was issued to you when you were under 16 years of age, if it has been more than 15 years since your last passport, if you cannot document a name change or if you are renewing for a minor. Download and fill out Form DS-11: Application for a U.S. Passport. Gather your proof of citizenship, identification and two passport-sized photos. Additional documentation is necessary to renew passports for minor children. Include the applicable fee. Make an appointment with the Minneapolis Passport Agency by calling 1-877-487-2778 if you need to renew your passport for travel within two weeks or to obtain a foreign visa within four weeks. Arrive for your appointment at least 15 minutes early. Bring your application materials, your appointment confirmation number or social security number.
