Go to any post office branch that has a Passport Check & Send service and request a Lost or Stolen Notification Form (LS01 application); request an LS01 application form online (see link below for LS01 Form and have it mailed to you (or complete and submit the form online and the IPS will send it to you for your signature); or call 0300 222 0000 and ask the Identity and Passport Service (IPS) to send you the form.
Complete the form and sign it on line 9 and have it countersigned on line 10 by someone who has known you for two years, is over 18, holds a current Irish or British passport and is willing to write his passport number on the form.
Get two passport photos and have the person who countersigned your application sign the photos as well.
Secure other documents you may need such as birth certificates, marriage certificates or adoption records as applicable.
Report a lost passport to the local police if you are abroad. Report the lost passport to the UK consulate, high commission or embassy. They will give you an LS01 form to complete. They will also notify the IPS to cancel your passport. To find the nearest Foreign Commonwealth Office (FCO) call +44 (0)20 7008 1500 (0870 606 0290 within the UK) from anywhere.
Get your replacement travel documents (to get back to the UK) from the FCO. These do not constitute a passport, but they serve as a temporary substitute so you can travel home. When you return to the UK, follow the steps above.
