      Download forms DS-11 -- Application for a U.S. Passport and DS-64 -- Statement Regarding a Lost or Stolen Passport from the U.S. Department of State website.

      Complete the DS-64 form with information pertaining to how and where your passport was lost. Ensure that all information is accurate because this form will be kept on file permanently with the Department of State.

      Complete the DS-11 form with your personal information. Be sure to complete Question 21, the section regarding your previous passport.

      Bring both completed forms along with your certified birth certificate, valid driver's license and two recent color passport photographs to one of Tucson's Passport Acceptance Facility locations. There were 13 locations in 2010, 11 of which are equipped to take passport photos on site.

      Pay the appropriate application fees. The standard fee provides for regular processing, which takes an average of four to six weeks. Expedited processing in two to three weeks bears an additional charge.
