How to Replace a Mutilated Passport
The United States government considers passports that have normal wear and tear, such as some scratches or bending, as acceptable. However, if there is water damage, a significant tear in the book, pages missing or a hole punched in the book, you must replace the passport as soon as possible. Otherwise, it may not be accepted when you attempt to travel internationally.

- Passport
- Birth certificate
- Driver license or other government-issued ID
- $170 cash or check

Fill out form DS-11 with your name, address, social security number, height, weight, and other information requested. This form is available at many post offices, passport offices, and online.

Affix the passport photo to the area indicated on form DS-11.

Sign the passport using a black or blue pen.

Take the completed form, along with the mutilated passport and $170 cash or check to the nearest passport agency.
