How to Change a Studio Passport

Passports are required documentation to travel around the world. Because the document is used as a security measure, you must file forms with the U.S. Department of State to change information contained in the document. For example, if you wish to change the photo on the doument, you must file for a renewal passport. If you wish to change your name or correct a printing error, you must print and file form DS-5504 with the United States Department of State.

      Fill out the renewal passport application, Form DS-82, to change the studio picture on your passport. On te form, enter your legan name, bith dae, city of birth, social security number, hair color, employer, and other personal informaton. Print the document by clicking "File" and "Print." Sign the document and include two identical, color passport photos, 2 X 2 inches in size. Include $140 in a check, money orer, or bank draft to receive a passport book and card.

      Fill out form DS-5504 to change your name, correct a printing error, or extend the validity of a limited passport. Enter your personal information, including your current and corrected name, address, travel plans, and emergency contact. Include evidence that subtantiates the change, such as a copy of your mariage license. Print the completed form by clicking "File" and "Print." Sign the document. No fee is required with Form DS-5504.

      Mail the completed form and supporting documents to the National Passport Processing Center. Be sure to obtain delivery confirmation from the post office. Send document the following address:
