How to Change a Surname on a Passport

Without a valid United States passport, citizens cannot travel internationally; if your surname changes, you must notify the U.S. Department of State to obtain a new passport. Attempting to travel internationally with a passport stating a surname you no longer legally use will result in denial of entry/exit into any foreign country. Fortunately, the processing of changing your surname on your passport is simple, especially if your current passport is less than a year old, and you can make the change free of charge. Otherwise, the name-change process requires renewing your passport, which requires paying a fee.

Instructions

  1. Passport Name Change

      Check the issuance date on the information name of your passport; if the date of issuance was less than one year ago, you need to complete form DS-5504 (see Resources) from the U.S. Department of State's passport website or from your local passport acceptance facility.

      Gather the following documents: your current passport, a certified copy of your marriage certificate, divorce decree or court order granting permission to legally change your name and one passport photo. The passport photo must meet the same requirements as the photo in your current passport.

      Mail form DS-5504 and all supporting documents (for regular processing) to:

      Your new passport is delivered to you six to eight weeks after your request is received by the U.S. Department of State.

      Expedite the processing of your passport to have it delivered to you two to three weeks after your request is received by writing a check to the U.S. Department of State in the amount of $60 and including it with form DS-5504 and your supporting documents. Mail to:

    Passport Name Change Renewal

      Check the issuance date on the information page of your current passport. If the date is more than one year ago you need to complete form DS-82 (see Resources) from the U.S. Department of State or any passport acceptance facility to renew your passport.

      Gather your current passport, a certified copy of the document supporting your new name and one passport photo, which must meet all passport photo requirements. Write a check to the U.S. Department of State in the amount of $110, which is the passport renewal fee as of 2011.

      Mail form DS-82, your renewal fees and supporting documents (using regular processing times) to:

      Your new passport is delivered six to eight weeks after your request is received.
