Complete form DS-5504, Application for a U.S. Passport: Name Change, Data Correction, and Limited Passport Book Replacement Form. This form requires you to include your personal information such as name, address, passport number, previous name and new name. This form must be completed perfectly to avoid any errors on your new passport. This is the form to use if your name has been changed in less than a year. If you have had the name change more than 1 year from issuance of your last passport, you must complete form DS-82 and pay the renewal fee. You may pick up these forms at your local post office.
Supply your marriage certificate or name change decree. You must prove your new name in order to have your name changed on your passport. Be sure to include this information the first time you submit your application to reduce delays. You must have an actual copy of your marriage certificate (they will send it back with your new passport) or a name change decree.
Include two 2-by-2 passport photos in color. Many places take passport photos, so just have your pictures taken and include these with your name change form.
Send your current valid passport. You must send your current passport with the other required documents to have your name changed. You will receive a new passport. The old one will be hole punched and returned to you.
