Download the Newly Wed and Civil Partner form if you will be changing your name upon your marriage or civil partnership and will be traveling outside of the UK immediately following the ceremony (see Resources section below).
Fill out the section that asks your current name and the name by which you will be known following your wedding. You will also need to fill out the date you will be married. You can complete this form up to 12 weeks before your wedding date.
Give the passport amendment form to the clergy person or other officiant who will be performing your marriage or partnership ceremony. The officiant will need to know both spouses' birth dates for his section of the form.
Bring the amendment forms and your current passport to the nearest Check and Send office, a passport service that is available at some UK post offices. Use the Identity and Passport Service's directory to find an office near you (see Resources).
Go to your nearest Check and Send outlet to amend your passport if you have already changed your name and need to reflect this change on your passport.
Bring your current passport, your official UK birth certificate, civil partner or marriage certificate, divorce decree or any other paperwork that states your new name and the reason for your name change.
Pay any fees associated with the name change amendment at the Check and Send office in British currency.
