How to Renew Your Passport After a Name Change

Whether you're getting married, divorced or simply decided to change your name, making the change can be a cumbersome task. It is important to remember to change your name on all important documents, including your passport. Passports may be forgotten if you have not traveled out of the country recently. After you have received your new social security card with your new name, you can apply for a new passport with your new name on it.

      Fill out Form DS-5504 if your current passport was issued less than one year ago. This is the U.S. Passport Name Change, Data Correction and Limited Passport Book Replacement Form.You can download it online or pick one up at a local acceptance facility such as a Post Office. You will also need a certified copy of your marriage certificate or official document stating your legal name change, you current passport and two passport-sized photos. There is no fee for a name change on your passport if it was issued less than one year ago, however you may pay for expedited passport processing if you need your passport in a hurry.

      Fill out Form DS-82 if your current passport was issued more than one year ago. This is the basic U.S. Passport renewal form. You will need to fill out this form if your passport has expired as well. You will also need to submit your current passport, two passport-sized photos and your certified marriage certificate or official documentation of your legal name change. The passport renewal fee was $110 as of 2010.

      Mail in your application, identifying documents and applicable fee in a padded envelope. Passport renewals and name changes can be mailed in to the National Passport Processing Center. Make checks payable to the Department of State. Write a separate check for expedited processing, which cost $60 in 2010. Make sure you label the outside of the envelope "EXPEDITE." All passports are processed at the National Passport Processing Center in Philadelphia, Pennsylvania. You can also take your application and documents to your local post office, pay any postage fees and they will send the package for you. Expect your new passport to take four to six weeks for regular processing and two to three weeks for expedited processing. This includes overnight delivery of your new passport to your home.
