Complete the required passport application forms. The necessary forms are available on the Passport Canada website. Complete the forms online or print them to complete the forms by hand. Note that name changes disqualify you from using the Simplified Renewal Forms.
Provide proof of identity. You will need to show at least one of the following identifiable documents: provincial driver's license, provincial health care card, federal identification card, old age security card, employee identification card or Canadian passport. You will also need to provide proof documenting your name change, such as a certificate of marriage or court papers approving a change of name.
Provide proof of citizenship. If you were born in Canada, you will need to provide a Canadian birth certificate, issued in Canada by the province or territory of your birth, or a certificate of Canadian citizenship. Those born outside of Canada will need to provide either a certificate of Canadian citizenship, certificate of Canadian naturalization, certificate of retention of Canadian citizenship or certificate of registration of birth abroad. All documents shown must be originals. Photocopies will not be accepted.
Supply two identical passport photos. There are quite a few guidelines and restrictions that apply to Canadian passport photos. These restrictions are important to note because any unacceptable photo will result in your passport application being rejected. A complete list of guidelines and restrictions is available on the Passport Canada website.
Find a guarantor. To be eligible, a guarantor must hold a five-year Canadian passport that is valid, have known you personally for at least two years, be a Canadian citizen at least 18 years old, and have been at least 16 or older when issued his own passport.
Have your guarantor verify the information on your application form by completing and signing the "Declaration of Guarantor" section. The guarantor must write, "I certify this to be a true likeness of (your name)" on the back of one of the passport photos and sign. In some cases, the guarantor will also be required to sign a copy of the documents that verify your identity.
Pay the passport application fee. If applying in person, you may pay by check, money order, credit or debit card. If applying by mail, check, money order or major credit cards are all acceptable payment methods.
Submit your application. In five weeks or less, your new Canadian passport with your new name will arrive.
