      Print and complete the interactive passport application, which is available on the Passport Canada website. Be sure to include your name, as written on your proof of citizenship, and your birthplace.

      Gather your required documents, including two passport photos, proof of citizenship and proof of identity. Either your Canadian birth certificate or Certificate of Canadian Citizenship fulfill the requirement for proof of citizenship. Your proof of identity could be an Ontario driver's licence, health care card or other provincial or federal identification card.

      Have your guarantor fill out the "Declaration of Guarantor" section of your application form. Your guarantor must also write "I certify this to be a true likeness of (your name)" on the back of one of your passport photos and sign their name. Your guarantor must be a Canadian citizen over 18 years of age, have known you for at least two years and hold a valid five-year Canadian passport.

      Take your documents to the nearest Ontario passport office or mail your required documents directly to Passport Canada. Be sure to include your visa processing fee. Processing usually takes between 2 and 4 weeks.
