Locate the nearest passport identification center. Issaquah has three local centers that can process passport applications. You may choose from the Issaquah City Clerk's Department, the King County Court District House and the Issaquah branch of the United States Post Office. Check hours before you go. As of early 2011, the Clerk's Department takes applications on Tuesdays and Wednesdays between 10 a.m. and 4 p.m.; the King County Court District House takes applications on weekdays between 8:30 a.m. and 4 p.m. and the Post Office takes applications on weekdays between 9:30 a.m. and 2:30 p.m. The Post Office location processes applications by appointment only.
Fill out Form DS-11: Application for a U. S. Passport. You can obtain this form onsite or print one out from the U. S. State Department website beforehand. Do not sign the application until instructed to do so by the acceptance agent.
Submit identification material to prove your U. S. citizenship. Acceptable documents include: a previously issued passport; a certified birth certificate issued by the city, county or state; a consular report of birth abroad; a naturalization certificate or a certificate of citizenship. Provide original documents, not copies -- all documents will be returned to you.
Submit proof of identity. Acceptable identification includes a previously issued passport, a naturalization certificate, a valid driver's license or a current government or military ID. You may provide photocopies of these documents. Provide copies of both the front and back of the ID on plain 8.5-by-11-inch paper.
Submit the applicable fees. As of January 2011, the fee for an adult passport booklet is $110, payable to U. S. Passport Services, plus a $40 execution fee payable to the location that processes your passport application. The Issaquah City Clerk's department only processes cash or checks -- other Issaquah locations will accept debit or credit cards.
Submit two identical passport photos. Make sure the photos meet all quality requirements. They must measure 2-by-2 inches, be in full color and printed on thin photo-quality paper. They must be taken within the past six months of applying and display your current appearance, in normal street attire, not uniforms. Both the Issaquah City Clerk's department and the Issaquah Post Office locations offer onsite passport photo service. If you apply at the King County Court House, you must obtain your photos elsewhere. Many businesses in or around Issaquah offer passport photo services, such as Star Images Photography, Ragasa Photography or the UPS Store in Renton, Washington.
Sign the application form when instructed to do so by your acceptance agent. Allow six to eight weeks processing time for your passport to arrive.
