Navigate to the Embassy of Lebanon's "Consular Affairs -- Jurisdictions" website. Review the map on the web page and click on the region in which you live to learn which Lebanese Consulate or Embassy serves your region.
Request in writing two Passport Registration Forms from the Consulate or Embassy that serves your area. Include a self-addressed, stamped envelope along with your written request. The agency will mail the two forms to you in the envelope that you provide.
Place your right thumb print on both Passport Registration Forms in front of a notary public. The notary will need to notarize the prints by completing a notarized statement. The statement is available on the Embassy of Lebanon's website. Make sure that the notary is aware that he needs to sign this particular notarized statement in advance of your meeting with him.
Obtain three two inch by two inch, recent passport sized photographs of yourself.
Complete the Passport Application form. It is available on the Embassy of Lebanon's website. Provide your full name and your mother's full maiden name. Indicate whether you are married and, if applicable, the full name of your spouse. Additionally, the form asks for your birth date, sex, occupation, contact information in the United States and contact information in Lebanon. The form also requests information about your current or last Lebanese passport; you should write down your passport number, the day it was issued to you and its expiration date. Lastly, provide information about your Lebanese Civil Status Record and your United States residency status. Sign the form and make a copy of it.
Place the two Passport Registration Forms, the notarized statement, the photographs and two copies of your Passport Application form in an envelope. Include your original Lebanese identification card, your old Lebanese passport, a photocopy of your old passport and a money order for $40 payable to the Embassy of Lebanon. Along with the documents above, put a self-addressed, stamped envelope in the package so that the Embassy or Consulate can mail your new passport to you. Send the entire package to the same Embassy or Consulate from which you requested and received the Passport Registration forms.
