How to Report a Missing Passport
American passports, issued by the U.S. Department of State, are documents which enable holders to travel to other countries and serve as proof of identity and citizenship. If a passport is lost or stolen, the holder must alert the Department of State. Doing so will assure that the passport is invalidated, a necessary step before it can be replaced with a new document. While there is no fee or sanction for reporting a lost or stolen passport, replacing it requires a separate filing and fee.

Access the DS-64 form on the U.S. Department of State website and print it out. You can also obtain this form through a local passport agency or passport acceptance facility.

Fill out the DS-64 form completely. Make sure to check "Lost" or "Stolen" on the top of the form. Sign and date the bottom.

Send the completed form to the U.S. Department of State Passport Services, Consular Lost/Stolen Passport Section, Attn: CLASP, 1111 19th St., NW, Suite 500, Washington, DC 20036.
