Obtain a copy of Form DS-82, Application for a U.S. Passport by Mail. Form DS-82 can be obtained online (see the Resources link below), at a local passport acceptance facility or passport agency.
Sign and date Form DS-82. Include with your form two passport photos, a marriage certificate or court order if you've changed your name, and a check for the $75 passport renewal fee. Passport photos must be of your full face on a white background, between one and one and 3/8 inch from the bottom of your chin to the top of your forehead, in regular clothes, and with no headgear or glasses, unless they are prescription. Passport photos must be in color.
Mail Form DS-82 along with two passport photos, your current passport and your renewal fee to:

You will receive your new passport by mail four to six weeks after submitting your application.

Obtain Form DS-82 from your local passport acceptance facility and fill out the appropriate information.
File Form DS-82, your current passport, two passport photos, fees and any additional information that may be required for your circumstances, such as name change information, with employees at your passport acceptance facility. Your application will be submitted and your passport will arrive by mail in four to six weeks, or in two to three weeks if you chose to expedite your passport renewal forms.
