How to Renew a Passport in Arizona

United States citizens must have a valid U.S. passport to travel internationally. The United States Department of State provides two forms for passport renewal. The forms are available online, or from local passport agencies or facilities. In Arizona, individuals can request a copy of the forms from the Clerk of the Superior Court's Office, Passport Services or Customer Service Centers. Depending on the conditions of renewal, an applicant can renew their passport by mail or in person in accordance with passport renewal guidelines.

      Review the passport renewal terms and conditions provided on the United States Department of State, Bureau of Consular Affairs website. You can renew your passport by mail if you can submit your passport with your application, your passport has not been damaged, you were 16 years or older when it was issued, it was issued in the past 15 years, and it was issued in your current name or you can provide legal proof of a name change. Applicants who do not meet this criteria must submit their passport renewal in person.

      Obtain a copy of Form DS-82: Renewal Application for a United States Passport, if you meet the passport renewal requirements by mail. There are three ways to request the form. Send a self-addressed stamped envelope to the Arizona Clerk of the Superior Court, Passport Services requesting a copy of the form; visit a Customer Services Center; or download the form from the United States Department of State, Bureau of Consular Affairs website.

      Complete form DS-82. Obtain two color passport photographs of yourself. Submit the photographs, your current passport and the application fee according to the Form DS-82 guidelines. If your name has changed, submit copies of legal documentation such as a certificate of marriage or court order. Sign and date Form DS-82. Mail your application in a padded envelope to the National Passport Processing Center address found on form DS-82.

      Complete Form DS-11 if you have to renew your passport in person. Find your local Customer Services Center. Bring proof of United States citizenship, two color photographs, proof of identity and the application fee with the form. Sign and date the form at your Customer Services Center.
