Visit the Department of State's passport renewal website.
Find Form DS-82, Application for a U.S. Passport by Mail. Click on "Fill Out Form DS-82 Online" if you wish to use this method, or "Print a Blank Form DS-82" if you plan to fill it out by hand. You will still have to print out and mail the form even if you complete it online; you cannot submit it electronically.
Print out the blank form or complete the form online. You must provide your current name and address. Other required information includes your social security number, the current passport number, the date the passport was issued and emergency contacts. Print out the online form if you completed it electronically.
Attach two recent color photos to the form. For detailed photo requirements, see Resources.
Sign the completed form. Mail it with the photos, your current passport and the renewal fee to the National Passport Processing Center, P.O. Box 90155, Philadelphia, PA 19190-0155. For information on fees, see Resources.

Find Form DS-11, Application for a U.S. Passport. Print it out and compete it. Do not sign it.
Check the passport website to find your local passport agency. If you need the passport to travel within two weeks, call the National Passport Information Center at 877-487-2778 and make an appointment.
Bring the completed, unsigned application to the passport agency, along with your current passport, passport photos, proof of identification and legal proof of your name if you have changed it. If you do not have a passport, bring evidence of U.S. citizenship. Bring a photocopy of your identification and proof of citizenship and name change.
Present your application, documentation and photos. Sign the form in the presence of the clerk. Pay the fee.
