Apply in person at a passport acceptance agency if your passport has been damaged, lost, or stolen. Also go in if your passport has expired and you received it more than 15 years ago. You'll also have to make a personal trip if your passport has expired and you were younger than 16 when it was issued to you, or if your name changed since your previous passport was issued to you, and there is no legal documentation showing your name.
Bring the following with you to a passport acceptance agency located in Chicago: Application form DS-11 ( the form should be filled out and you shouldn’t sign it until the agent tells you to do so), proof of citizenship, proof of identity, filing fee, two passport size pictures and your social security number. All documents will be returned when your passport is issued.
Apply by mail if you meet all of these requirements: the old passport is not damaged, you were at least 16 when you received it, It was issued to you within the last 15 years and your name is the same as on the old passport.
Fill out the application form DS-82. Make sure you sign your application and put a date on it.
Attach your old passport, two identical passport pictures, the passport fee (there is an additional fee for expedited service), and legal documentation of your name change if this applies to you (no copies).
Send it to the address on the form in a padded envelope.
Apply through a passport agency only if you need your passport within 14 days.
