Fill out form DS-11, the Application for Passport. You can download and print this form at the U.S. State Department website, or you can get it at any passport office. You must take the completed form to a passport office or passport acceptance facility and apply in person.
Find a location to present your application on the passport acceptance facility search page of the Department of State website. Bring a birth certificate, a form of payment and 2 photos of yourself to replace your stolen passport.
Pay the application fee and the Security Surcharge. The fees are non-refundable and are subject to change. Check with your local passport office for current rates. If you need to replace your stolen passport quickly, you can pay for expedited service.
