Visit the Department of State website. On this website you can download the appropriate passport form whether it be DS-11, DS-82, DS-5504 or DS-4085. There is a form wizard on the site that will help you determine which form is the right one for you as well as estimate your payment. The wizard will walk you through the steps and help you fill out the information. Then, you can print the form already completed.
Print the form. You will need to print the form ensuring there is enough ink in the printer to print an easily legible form. Also, only print one sided pages of the form. Use only black ink on white paper or it will not be accepted. You should use medium weight paper that is 8 1/2 inches by 11 inches with no holes of any kind. Shiny paper, thermal paper, special inkjet paper, and the like will not be accepted.
Watch for the photo size. When you download and print the passport application the photo box may be smaller than 2x2. You must still submit the correct size passport photos that are 2x2. Just because the form you printed has a smaller size does not mean smaller photos will be accepted.
