Getting Your Passport Application Online

  Go to the U.S. Department of State website (see Resources section).

  Click the button for "PRINT A BLANK FORM DS-11" if you wish to print a blank form, and go to Step 4. Click the button for "COMPLETE FORM DS-11 ONLINE" if you wish to fill out the form on your computer and then print it.

  If you wish to fill out the form online, review the notices and disclaimers, then click "Submit." Then click "Continue" under "Apply for a Passport Book." Type in answers to each question and check that all information is correct before printing.

  Print the form in black ink, printing only on one side of the paper.

  Fill out the form using a black pen if you did not type in your answers on your computer.

Getting Your Passport Application In Person

  Use the U.S Department of State website's office search page (see Resources section).

  Type your ZIP code in the appropriate box and click "Search." A list of locations where you can get a passport application will appear.

  Click on a location to get more information, such as hours of operation.
