Download the application from India's embassy (see the link in the References section) and print out a hard copy.
Fill out the application using a blue or black pen. Include all required information for questions one through ten. Use each box for a separate letter for answers to questions such as Mother's Full Name. Attached one of your passport photos, measuring three and one-half centimeters by three and one-half centimeters, in the designated box at the top right corner of the first page.
Write the answers for numbers 11 through 18 on the designated lines. Be thorough and honest in your responses to every question.
Make an appointment at your local embassy or passport center. If you are in the United States, you must visit the India embassy there. Sign and date item number 19 in front of an official and place your fingerprint on the paper. Be sure to take your photo identification, proof of residency and educational information with you.
Answer all questions on pages five and six of the duplicate copies. Write the answers clearly on the lines provided. Attach your two remaining passport photographs where designated in the top right corners.
