Download and print the Nicaraguan passport application form in either Word or PDF format from the website of the Nicaraguan Ministry of Exterior Relations.
Use the glue to stick one of your passport photos in the passport-photo sized space in the top left corner of the form. You may need to include several copies of the same photo depending on your individual circumstances, but only one should be stuck into place.
Place an "X" in one of the five boxes listed under the "Tipo de Pasaporte" (Passport Type) in section "A" of the form. Reading downwards in order, your choices are "Ordinary," "Diplomatic," "Service," "Official" and "Provisional."
Complete section "B" of the form which requires you to enter all your personal details in capital letters. The first line asks for your surname, the second your given name and the third your date and place of birth.
Write your current passport number -- if you have one -- at the start of the fifth line, followed by the area and date where you registered. Add your ID number and date on the next line and fill in the final four lines with your gender, profession, address, contact number and any distinguishing facial features.
Complete the three lines of section "C" by entering your parents' names on the first two lines as well as their nationality. On the third line, enter the details of your spouse -- if any.
Pay the $55 application fee and write the receipt number and dollar amount on the two lines below the "Recibo del Min. De Relaciones Exteriores" heading in section "D." If you're not applying for a passport at a Nicaraguan embassy or consulate, then you will need to pay the appropriate fee at a bank or government office in Nicaragua and enter the details in the first two parts of this section.
Complete section "E" only if you're a foreign citizen applying for a Nicaraguan passport. You will need to enter your visa details here.
Sign and date the form in the space that says "Firma del Solicitante." Avoid writing or marking anything in section "F" as this part of the form is for official use only.
