How to Fill Out Question 10 on the U.S. Passport Renewal Form

So, it is time for you to renew your U.S. passport, and you have begun to complete your U.S. Passport Renewal Application for Eligible Individuals, Form DS-82. You reach question 10, and it requests a passport book number and a passport card number. Perhaps you only have a standard passport book and do not even know what a passport card looks like. How should you answer question 10? Follow these steps and you will soon have your renewal application in the mail on its way to the National Passport Processing Center.

Instructions

      Determine whether you are renewing a passport book or a passport card. A passport book is the standard blue passport, while the wallet-sized passport card resembles a driver's license.

      Proceed to question 10 and enter your name exactly as it is printed on your passport book and/or your passport card, depending upon which one you wish to renew. If you have changed your name legally since your passport was issued, you will enter that information in question 11 and attach documentation to your passport renewal application.

      Enter your most recent passport book number and issue date, if you are renewing your passport book. Locate this passport book number, described as the "passport number", on the upper right hand corner of the first page of your passport book to the right of of your photograph.

      Enter the most recent passport card number and issue date, if you are renewing your passport card. Locate the passport card number on the upper right hand side of the front of your passport card. The passport card issue date is located on the front of the card to the left of the expiration date.
