How to Obtain a U.S. Passport for a Newborn

The steps to obtain a U.S. passport for a newborn are the same as those for all minors under the age of 16. All minors including newborns must have their own passport when traveling internationally by air. There are some instances where newborns are not required to have a passport, such as when traveling to Canada and Mexico.       Obtain and complete Form DS-11 Application for U.S. passport. You can obtain the DS-11 by visiting the U.S. Department of State's website or from a local passport acceptance facility or passport agency. Do not sign the application if you choose to fill it out at home. You must sign it in front of an acceptance agent.

      Obtain passport photos of the newborn applying for the passport. You must submit two identical, 2-by-2 inch photos taken against a white background. Passport photos can be taken at home if they meet the criteria or can be obtained from professional photographer services.

      Gather all necessary documents and information including the DS-11, your newborn's birth certificate as proof of citizenship, a document showing evidence of relationship between the infant and parents such as their birth certificate with both parent's names listed, identification of parents by acceptable document, and a photocopy of the identification document. Lists of the approved documents are available on the U.S. Department of State's website or by contacting a local agency.

      Fill out appropriate documents for parental consent. If both parents cannot accompany the minor to apply for the passport certain documentation is necessary to provide parental consent to obtain the passport. If both parents can accompany the minor to apply no addition forms are necessary but they both must sign the application in front of an acceptance agent.

      Travel to a local passport acceptance facility or passport agency to submit application, photos, supporting documents and application fee. Sign the application in front of a passport acceptance agent. All documentation will be mailed to the U.S. Department of State and the passport and documents will be returned to you once the passport is issued.
