Locate the Cuban Consulate office in your jurisdiction by searching the government section of a phone book or an online source such as Embassies Abroad (see Resources). Applying for a Cuba passport requires a personal appearance as some documents require a signature in the presence of a Consular.
Complete a passport application at the Consulate. The application is called the planilla de solicitud de pasaporte. It is also available for download (see Resources), but cannot be signed until your appointment.
Provide 4 identical passport photographs 2-by-2 inches in size, and tape one to each page of your application forms. The photos must show a frontal view of the face, without eyeglasses or anything worn on the head.
Present an original birth certificate indicating the exact location of your birth. Also provide your Cuban identity card, or carné de identidad, or a valid driver's license as personal photo identification.
Submit your application and passport materials , then pay the service fees. They will differ according to location and the currency of your country. Provide a stamped, self-addressed envelope to have your passport delivered in the mail at an extra cost, or collect it from the designated office within one to two business weeks.
