Enter your address and postal code on the United States Department of State website to find the nearest passport application acceptance facility. The Milwaukee County Clerk's Office is one such location, and it accepts applications on weekdays between the hours of 8 a.m. and 4 p.m.
Print out the the application form from the U.S. Department of State website, and fill it out. You must present it in person at the acceptance facility.
Make a copy of both sides of your valid government-issued ID on 8 1/2-by-11 inch white paper. Present this at the time of application alongside the original ID.
Write out two checks in the appropriate amounts to the U.S. Department of State and the acceptance facility. As of 2010, you must pay $110 to the U.S. Department of State and $25 to the acceptance facility.
Have two full color passport photos taken, or have them taken at the application facility. They should measure 2-by-2 inches.
Bring all the necessary documents to the application facility. After approximately 6 weeks, your passport should be available.
