Fill out form DS-11, found on the State Department's website. You can print the form out and fill it in or type your answers online before printing it out. Don't sign the form until you are instructed to do so at the acceptance facility.
Take a passport photo following the guidelines found in on the State Department's website. You will need two copies of the photo.
Gather and copy your identification. You will need a Florida photo ID and proof of citizenship, such as a birth certificate or naturalization card.
Make an appointment at the Miami Passport Agency or one of the acceptance facilities listed on the State Department's website.
Bring your application, identification, payment and photos to your appointment. Sign the application when instructed to do so.
Pay the passport application fee and the execution fee in separate payments. You can write checks ahead of time made out to the Department of State or pay by credit card at a passport agency. Acceptance facilities won't take credit cards, but sometimes will take cash. Verify the acceptable payment methods with the specific location where you are applying.
Return to the facility to pick up your passport.
