Fill out form DS-11, found on the State Department website. The form must be completely filled out, except for your signature. You will have to sign the application in person at the courthouse.
Gather documents to prove your identity and citizenship. A Florida or U.S. government-issued ID will prove your identity. Your birth certificate, a naturalization card or citizenship certificate will prove that you are an American.
Photocopy your ID documents to submit with your application and your original documents.
Take a picture at a photo agency, drugstore, post office or anywhere else that takes official passport photos. View the "Guidelines for Producing High Quality Photos for U.S. Travel Documents" page at the State Department website. You will need to submit two identical copies of the photo.
Make an appointment to turn in your application, fee, identification documents and photos at the Osceola County Courthouse. The courthouse is located at 2 Courthouse Square in Kissimmee.
Write a check made out to the Department of State or submit a credit card payment for the application fee and the execution fee. The current list of fees can also be found at the State Department's website.
Sign your application when instructed to at the courthouse acceptance facility.
Return to the courthouse to in Kissimmee to pick up your documents and your passport when instructed.
