Call the National Passport Information Center at 877-487-2778. The State Department will issue an emergency passport in cases of serious illness, injury or death of an immediate family member when travel is required in 24 to 48 hours. Schedule an appointment at the Miami Passport Agency in the Omni Center on Biscayne Blvd. in Miami. If no appointments are available through the automated appointment system, speak to a Customer Service Representative and explain your situation.
Assemble the required documentation for an emergency passport. You will need Form DS-11, the Application for a U.S. Passport, your birth certificate to prove U.S. citizenship, your driver's license to prove your identity, and two recent color photographs. You will also need documentation that proves your need for an emergency passport. This includes a copy of your travel itinerary and tickets you have purchased to fly.
Go to the Miami Passport Agency. You will not be admitted more than 15 minutes before your appointment time or 15 minutes after your appointment time. Pass through the building security metal detectors and take the elevator to the third floor. When you check in at the Information Desk, give your social security number or appointment confirmation number to the attendant to verify the appointment. You will receive a queue number and must wait until you are called.
Present your application, identification material, photographs and documentation for the emergency passport to the Customer Service Agent. As of 2010, you must pay a fee of $165 for a passport book and passport card, $135 for a passport book, or $55 for a passport card used to enter the U.S. at land crossings or seaports in Canada, Mexico, Bermuda and the Caribbean. You must also pay a $60.00 fee for expedited service. The Miami Passport Office can issue a passport card immediately. If you need to travel by air and require a passport book, you must also pay a $14.96 overnight delivery fee to have your passport delivered to your home or business the following day.
