Fill out a passport application form No. 1. You can download this form at the Government of India Ministry of External Affairs website. You can also pick up a paper application at any passport office, district passport center and Speed Post centers throughout India, according to the Ministry of External Affairs website. When filling out the application, you must only use a black or blue pen, write in capital letters and write neatly inside of the boxes. Leave a blank space between each word. Attached to the application form is the personal particulars form (PP), which is for police verification. You will need two copies of the PP form if you have lived in more than one place in one year. If applicant is illiterate, they should have someone fill out the application on their behalf and place their thumbprint in lieu of a signature on the application form.
With the application, you will need to submit a proof of address such as an electric bill, proof of date of birth such as an Indian birth certificate, a citizenship document if the applicant is a naturalized citizen, government employee identification if you are a government employee, and any paperwork if applicant was ever deported to India or repatriated at the government's cost.
You must include three passport-sized color photographs with your application. In the picture, the person must be facing forward and should not be wearing any sunglasses. The picture should have a light, preferably white background. Attach one copy to the application form and one copy on both PP forms.
The fee for a new passport for adults as of 2010 is Rs. 1000, about $22 US for a 36-page passport and Rs. 1500, or $33 US for 60-page passport. A new passport for minors is Rs.600, which is about $13 US. Duplicate passports in case of a lost passport are Rs. 2500 and Rs. 3000, about $55 and $66 US, respectively. Fees may be paid by bank draft (check) or cash.
