Visit a pharmacy or department store to take two passport-sized photos. This is a specific size and has requirements of how much of your face must be exposed. Take this picture with a professional so it meets standards. Attach these photos to the first page of the application.
Fill out your basic information on the passport application. This includes your legal name, social security number, home address, any names you have been known by in the past (such as your maiden name) and your parents' information. Use a dark pen and print each letter in a separate box so it is clear to read. Accuracy and clarity are important.
Fill out the personal information about yourself. This includes height, weight, eye color, occupation, permanent address (if different from mailing) and phone number.
Choose an emergency contact to go on file with the U.S. Embassy. This should be a person you are close to and will be able to help if you run into problems. A good emergency contact is someone like a sibling (who is over 18), parent or spouse.
Write your travel plans. This includes how long you are leaving the country and where you plan to go. You do not need to go into detail, but provide enough information so it is clear you have a planned trip. Finally, mark if this is your first passport and if you have a passport card. Be honest and truthful; officials could deny your application if any information is false.
