Download and print the Jamaican passport application form. Complete the application in pen, following the directions to each section closely. Have a notary public sign, stamp and verify your identity and occupation in Section G of the form. Section G contains the official certification which must be completed if you are sending your application by mail.
Collect the required documents for submission with your completed application. You will need your original birth certificate, proof of any marriages, divorces or adoption if applicable, and a valid official photo identification. Accepted photo IDs are a Jamaican driver's license, a national voter's ID card and work permit. Or you may submit one of the following documents from your country of residence: proof of citizenship, driver's license, passport, or any identity document issued by a federal, city or state government. Also include two passport photos, with one of them notarized by the same notary who verified Section G of your application. Minors will also require parental identification.
Submit your application and any additional required documents to the Jamaican consulate or embassy nearest you. First-time applications may be submitted in person to a consular official or by mail to one of three locations listed by Jamaican consulate of Chicago. Include the appropriate fees with your application.
Contact the Consulate General of Jamaica four weeks or more after your application has been submitted to request an update on its status. The average wait time for a passport is four to six weeks.
