Download and fill out the online application from the Indian Embassy or nearest Indian consulate website.
Print the four-page application and fill in the areas that you cannot complete online. Write legibly by hand, using capital letters and a blue or black pen.
Take passport photos with a white background to submit with your application. Glue one of the photos to the space on the application and staple the rest to the last page of the application. You will need four photos if you are sending the application to Washington, D.C.; three are required for the Houston and New York consulates and only two are required at the San Francisco Consulate. Check your local consulate for the number required.
Write your signature next to the affixed photo, below the declaration in column 26 and in the boxes in column 29.
Dip your thumb in a pad of ink and place your thumb print next to your signature in columns 26 and 29.
Make photocopies of the first five pages and the last two pages of your current Indian passport. Submit these pages along with the current passport.
Make a color photocopy of your U.S. residency document. This could be a green card, valid work or residence visa or employment authorization card.
Notarize the residency document if you submit the application by mail.
Photocopy your driver's license or any lease or utility bill with your current address. Send a state ID if you are over 60 years old and do not have a lease or utility bills.
Print an affidavit from the Indian embassy or consulate website and have it notarized if your Indian passport has expired.
Obtain a certified check or money order made out to the Indian embassy or consulate closest to you. The fee will vary based upon the size of the passport required. Find the appropriate fee for your application on the embassy's website.
Send the documents and application form, along with your current passport, to the embassy or the nearest consulate by certified mail. Address the envelope to the "Passport Cell" of the consulate or embassy.
Enclose a postal fee or a USPS Express Mail paid envelope with your return address.
Locate the appointment date in the extreme right of the first page of the application, if you are applying in person at the consulate or embassy. This automatically generated appointment date ensures that the consulate will have processed your online application by the time you appear with your supporting documents.
Appear for an appointment at the embassy or consulate on or after the date stated on your application form. Come with your printed application, documents and passport in hand.
